In the first half of my life, I lived in the country with nationalized health care and limited private options. If one wanted to have good care in the nationalized hospital, a visit to the private office of the doctor in charge of the same patient in the hospital was helpful.

You are not correct that Americans do not debate health policy. They do, but major media outlets and politicians ignore it. I posted at least a few articles on Medium.
